We participate with most of the insurance companies including Medicare, CareFirst Bluecross / Blueshield, United Healthcare and Aetna. Please feel free to contact our office to confirm that we are in-network with your insurance carrier. As a convenience to our patients, we will submit itemized bills to your insurance carrier.
Upon your first visit with us we will require a copy of your current health insurance card along with your physicians order/referral. Please let us know of any changes that may occur with your coverage. If your insurance company notifies our office that you have a higher out of pocket expense, we will bill you accordingly, and payment is due upon receipt of the billing statement.
CO-PAY & DEDUCTIBLES
Co-pays are due at the time of service. If you have a percentage that is owed versus a standard co-pay this will be estimated and you would be required to pay that as well. We accept weekly payments for which arrangements must be made with our administrative assistant. If your insurance plan has an annual deductible, and you have not met your deductible, you will be responsible for your portion of the office visit as applicable. Please be advised that if we need to bill you for your co-pay there will be a $10.00 charge added.
